Appendix C Interview/Research Consent Forms Consent to Participate in Research: Campus Evangelical Organizations You are asked to participate in a research study conducted by Rebecca Kim, from the sociology department at the University of XX as part of dissertation research. You were selected as a possible participant in this study because you are a staff and/or a member of a campus evangelical organization and have some knowledge and experience with campus evangelical organizations. purpose of the study The purpose of the study is to gather general descriptive information on campus evangelical organizations and find out the major concerns and themes of such organizations. How and why students came to participate in campus evangelical organizations is also the subject of this study. procedures If you volunteer to participate in this study, I will interview you for approximately 30 minutes to an hour and a half at a location of your choice near campus and ask you questions regarding your involvement in campus ministry. potential risks and discomforts Your participation in this study is voluntary. You may refuse to answer any question that makes you uncomfortable. Moreover, your real name and identity will not be disclosed on public record. As a result, I do not anticipate that there will be any physical or mental discomfort as a result of participating in my study. 162 potential benefits to subjects and/or to society Through participating in this study, you will help contribute to our limited knowledge of campus evangelicalism in general and Asian American campus evangelicalism in particular. payment for participation There will be no financial payment for participating in the study. confidentiality Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission. Your interview may be audio-taped. However, you have the right to review and if necessary, edit the tapes. Only I will have access to the tapes and they will be erased after my dissertation is completed. participation and withdrawal You can choose whether to be in this study or not. If you volunteer to be in this study, you may withdraw at any time without consequences of any kind. You may also refuse to answer any questions you don’t want to answer and still remain in the study.
