Overview of Special Issue

The papers in this special issue of Perspectives in Biology and Medicine deal with the development of clinical research in the last half century as a basis for understanding the successes and current problems of the clinical research enterprise in the United States. In 1979, this journal published a symposium detailing the first evidence that, despite rapidly growing support of biomedical research, application of that research to patients was not flourishing. In our view, this problem has only worsened since that time. Now, 25 years later, we attempt to bring the problem of clinical or patient-oriented research into the contemporary context by publishing a number of essays under the broad headings of Historical Perspectives, Case Studies, Policy Issues, and Essay Reviews. Together, these articles provide a background for understanding the relationships between basic laboratory research and clinical research, and the factors presently impinging upon the public support of clinical research. We hope this issue will promote further discussion and will lead to greater support for patient-oriented research, and that this in turn will facilitate advances in medicine and health care.
